Self Management Symptom Progression
Use this form to track the progression of symptoms of mania and depression
for past episodes. The goal is to become an expert in the early
warning signs of mania and depression, so that you can use these warning signs
to identify the beginning of new episodes, and take action before you become
severely ill.
Below is a sample form to provide an example of how to chart
symptom progression in terms of changes in mood, behavior and thoughts.
